To qualify for financial assistance, you are a resident of Rock County, WI and in treatment for cancer. Current grant amount is $1,400 a year; this compensation amount is subject to funds available. These grants can be used for rent/mortgage, utility payments (water, electric, and sewer), phone/cable/internet bills, car insurance, health insurance, or car payments (see page 5 of the application).
Below is your application to download and print. If you do not have a printer:
1. When you go to your next oncology appointment, you can ask your caring team to print the application. Please remember that your oncology support team is very busy and this is not required. They may also be able to assist with having your oncologist complete your "Doctor Verification" Form (Page 4) of your application.
2. You can stop by our office at One Parker Place, Suite 320 to pick up an application. If someone is not in the office, there is a brochure holder in the lobby area that holds blank applications and other literature.
